Create a Java Swing sample app. (Easy)

I need to have a sample application created in Java Swing. This sample application does not have to really do anything. Its simply a "facade" that will be used by my application for demonstration purposes only. This sample application will visually mimic (as closely as possible) an existing application developed in .NET (see screenshot attached).
This should take a couple of hours and be easy money. Since the application does not have to do anything, it will be quick to test, accept and get you paid. The only thing i ask is to have to line things up and make the UI look professional. If you don't know what a professional UI looks like - PLEASE DON'T BID.
Take a look at the screen shot provided. I want my sample application to look exactly (as much as possible) like that. I need it quick, so be aware of the the tight deadline.
Other notes: The picture shows a tab control with People/Company/Other which can be removed if needed (since we will not be enabling the other two tabs. My preference would be to have the other two tabs visible but disabled.
