c# exe - hit a series of urls and store json response

We have 12 urls that we need to GET. they have basic http auth so we will need to authenticate from time to time. We want to store the responses (json) in a single csv for each url.
the response looks like this as a json response

we want to store the time date in a nice format for excel, and then in the next column we will store the numeric value. Each time we poll it we will make a new row in the CSV.
The exe you make should work like this
1-run it
2-get a gui to choose the csv for where the urls are (in the csv will be url, user,pass)
3-next we will be able to set a timer for how often we hit each of the urls. should be set in seconds like 10 or 3600 etc
4-it should write to the csv at each check, this way the csv file is not empty should the exe fail.
5- the name of the csv should be the url.
we will be running this exe for days/weeks, I am not sure if we need to run it as a service to make sure it is stable or not but I need to be able to run it for a very long time. This should work for roughly 10-50 urls. the response time appears to be roughly 500ms from where I am so I do not expect the app to have to deal much with timeouts. However if it gets a timeout, we should just add an entry to the csv row as timeout and the timestamp with it. this way we know what happened.
