I wold like to have patient surgery consent forms stored on DocuSign, so that my surgical counselor can simply pull the specific surgery consent form up on the iPad, the patient can sign, and check the various fields, and initial the pages, then be able to save the signed copy while not affecting the original blank surgery consent.
