Bootstrap form reconstruction

The company’s quote form pages need to be reworked to follow the new design and structure.
The new structure is using to Zend Framework but to edit the form is only affecting one .phtml page, a .js and/or a .css file.
The project is in bitbucket and an invitation will be sent for accessing the repository and it requires to create a bitbucket account.
The job is to convert the form on the following page [url removed, login to view] to the format on the page [url removed, login to view]
IMPORTANT: This is not simply a case of copying the form and pasting into a new template, You will need to manually re-construct the form to use Bootstrap and be mobile and desktop responsive.
Form element IDs should be the same.
Default values and select options should be the same.
Tooltip texts should be the same.
Error messages should be the same.
The validation should be replaced to google’s recaptcha.
There are already converted forms in the system and it is easy to follow the process.
Further specifications on the project installation will be provided once the butbucket invitation sent.
All Javascript must function as it currently does once project is complete.
