Contact Form and latest projects

Firstly I would need a register button which takes the user to a contact form with various fields e.g First Name, Last Name, Email, Date of birth. When they click submit, this form should be emailed to my email address so I have all the details.
Next I would need a button called About US: which takes the user to a page with some information about my company
I would need a 3rd button named "latest projects" I would update this daily with the latest projects I have with a brief description, If they meet the criteria, once they click apply, a page will come up with 2 blank boxes, Name and Email. Once they click submit, I will be emailed with their details and also the name of project they have applied for.
This app would be free of charge and also I would want to use the colour scheme and logo I am using on my site.
