I need a simple iPhone/ipad app done. I estimate 3 days of work but could vary depending on the expertise of the team.

The APP consists of 4-6 lines on the main screen. Pressing each line will call a number. User must be able to do inputs on each line containing color of line, name on line, number to call when pressed and add a picture. There must be a solution (ex. CAPTCHA) so that kids can't change user input. That is it.

Further I need a few more complex APPs but I would like to see the outcome of this before starting larger projects.
