I require a PHP / CSS designed and driven form for collecting personalisation details for customers orders
The form has two parts, depending on their first selection from a radio box
If they select "yes" as their first option they see one form, if they select "no" as their first option they see another page
Once the form has been submitted they will see a thank you page
The layout and design has been completed for the form and process - it just will need styling to match the design on the page
The form will be hosted on its own domain name (of which you will have access details)
