This small project involves setting up JavaScript popups in 10 html surveys. The popups will provide feedback information to participants on how they performed in the survey they completed. Minor calculations are required. Please read the attached zip for a full description of the project and go to [url removed, login to view] to see the current state of the website.
**Here are a few more notes after reading your responses:**
* There is no DB involved and the order of the questions will remain static.
* Checkboxes have been changed to radio buttons.
* Validation is required for the demographic details.
* No validation is required for the survey - I will validate the data using a statistics package. If no questions are answered then the submit button should be disabled. A confirmation message should appear if users submit when less than 25% of the questions have been answered.
* After the user submits the survey and the popup appears, the popup OK button should send them back to the Homepage.
* Competency Areas for each competency are now listed in each survey as a comment
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
