PHP Form With Checkboxes And Required Fields

I have attached a PHP form that I would like edited. Here are the edits:
1). The question entitled "Preferred day of the week for an appointment?*" has a menu of options as a dropdown BUT needs to be a checkbox that has a required answer(s).
2). The question entitled "Preferred time for an appointment?*" has a menu of options as a dropdown BUT needs to be a checkbox that has a required answer(s).
3). The question entitled "Best time to call?" is currently a checkbox BUT it is not written correctly. Please fix.
This should be very simple for a PHP programmer.
