Simple ColdFusion Picture Upload Form

We are running a contest on our website. Kids can upload photos to be entered into a draw.

We need a developer to create the form for ColdFusion. This form will collect basic information:

First Name

Last Name Inital

Grade

School Name

Each form field should have inline ajax validation.

Users can upload multiple files. I would like this to take advantage of the plupload library.

[url removed, login to view] which is a free cross browser uploader.

Once the form is submitted via AJAX the files should be uploaded and the form fields added to the database along with the file names.

Upon completion a success message should be displayed.

JQuery can be used for this form or JQuery plugins.

The server is NOT available for testing so it will have to be created on your environment.

If there is something that needs clarification please ask. Otherwise the deliverables should work as described.
