Wordpress JSON Feed Integration

Hi,
I need your help to create a JSON feed similar to - [url removed, login to view] Click on any "Find Schools" links, and you will see the feed in action. This is two part implementation.
1) Create an attractive form with below 3 field types as below:
- Field of study (arts, medicine etc.) - drop down menu
- Program (herbal studies, dental assistant etc.) - drop down menu
- Zip code - input field
This actual form in action including drop down menu lists is available on the next page once once you click on "find schools" link on above URL. (on top).
I will provide you these form elements - [url removed, login to view] to create above field types.
2) Fetch JSON result feed based on above pre-selected criterias and show up on blank page - [url removed, login to view] Attached documents will give you implementation overview.
