Simple 2 page "Reserve a Time" site

I am trying to save myself some time. Currently, I have a manual system for teams to sign up for practice space on 11/13 & 11/14. (this would only be used once a year!!!) However, here are the steps:
1. A team goes to the links and in the form box fills out information and selects time and hit submits, which sends me an email.
2. I then have to take email, open dreamweaver and file, put in team's name, upload the file to server and then send them a confirmation email (attached).
There has to be a better and automated way!!!
I just need to make sure I can do the following:
1. Edit teams and remove teams
2. System would need to make sure once a time is reserved, that another team can't select that time.
3. Edit dates and text

Before, bidding, PLEASE make sure you read everything and understand the project!!! I would also like to see this working on the developer's server, before putting files on mine... I will award project in 1 hours, so please read everything!!!
