Build an Registration form with referral link

I need build an small demo project with only this features:
Custom User model (django)
Users registration form needs:
-> CPF (its an document from Brazil (000.000.000-00)
-> referral_link (user will create one on registration)
-> miBank account (nullable) (format=000000-00)
-> Full Name
-> created_at_date
-> Active (boolean)
-> Cellphone (we will send some SMS if need)
-> Father_ID (who gives the link to him)
User will register accessing this link:
[login to view URL]{REFERRAL_CODE}
Another user will give and referral link to him, when he access, screen need show the form and an small message:
"Seu upline é: {EMAIL_FROM_USER}"
No special html page needs, can use default bootstrap or pure HTML.
We will custom it in a hard way.
