Create Responsive Boostrap html pages from PDF mockup

I have made a mockup of a mobile app that I would like to be converted to responsive bootstrap html.
Only the controls and the layout need to be defined. No functionality is needed as this will be added later. The website will be mainly for mobile use. It will be converted using PHONEGAP so it needs to be pure HTML, JScript and CSS.
Although the website will be mainly for mobile use it will also need to be presentable in a normal web browser so responsive layout needs to be considered.
The attached PDF shows the basic layout of each page. The style should be clean and clear.
The background will be either black or a dark image. You will need to allow for both possibilities when considering color scheme. The colors in the PDF are by no means fixed a dark theme is needed.
Many of the pages in the PDF are duplicated to show different design ideas. It will be up to you to decide which format is the most stylish.
All work will needed to be done live on my own server. You will be given ftp details so that I can monitor the progress.
The work will need to be complete within 5 days from project start.
