Form Builder (experience with Gravity Forms required)

We are looking for a Form Builder who is experienced with Gravity Forms and can create complicated forms quickly. Ideally experienced building registration (paid and unpaid) forms where categories are selected and personal information entered. Experience using conditional logic viewed favourably. Please submit examples if any.
The qualified person should have a very high score on Upwork.
About us:
We work with a number of different clients on web projects. We would like to trial working with the successful applicant on a single project - if it goes well, there will be room for more projects moving forward. We would estimate regular work of between 5 - 20 hours a week.
