Customize Sharepoint Survey

I need to be able to create a survey using SharePoint 2007 that will allow me to use images in it, so that I can ask questions based on the images shown. Also if I use a rating scale question, I need to be able to include range text for all options, and not just the three that SharePoint offers. This needs to be done as a webpart, so that I can just install it based on the given instructions by the developer. ## Deliverables The webpart must be able to create any of the surveys that Sharpoint actually has.
