Custom search box in widget area

I am looking to add a custom search box to the widget area on the right hand side of our blog: [login to view URL] (password: Recruitment1@) It will be similar to the job search on this website: [login to view URL] So users should be able to choose from the dropdowns and radio buttons. This will then take them to the job results pages on the [login to view URL] website. The only problem however, is that once the users submit the search, we need to add the expiry date to the url like so: [login to view URL]:Daily%20ExpiryDate%3E=2014/12/22 Otherwise it will not return any results. I would like to keep the same formatting as the other form on the website though: [login to view URL]
