Design a Microsoft Word Report Template

I am in need of a polished Microsoft Word Template to use for reports I am generating. This is a job for a Microsoft Word Expert as I require a style set up correctly with tiered headings, an index page, a title page, etc. I need the attachment supplied "Report Example 2014" duplicated using my own branding and can supply all images. Please only bid if you know Microsoft Word well and can do this sort of work. This is not a graphic design job where I just need a header and footer created. This needs to be a dynamic word template where I can change formatting options of the headers in the styles section and not have to edit each one individually. I will also need a landscape format page designed. More detailed instructions will follow for the successful bidder. This should be a quick job for someone who knows this software well.
