Android Form App -- 2

I'm in need of an app to basically fill in a form with some set fields and some conditional fields. In total it might be about 70 fields depending on the user input. Users will need to login with their own username and password and can be managed via a simple web portal. Specific users should be able to access all app functions and reports. It'll need to be organized in pages and sub pages and visually pleasant and clean. Some fields are drop downs some other are just text boxes or multiple choice. The app will need to take pictures, capture the GPS position as needed and the signature of the user. The app cannot rely on an always on internet connection but data will have to be uploaded as soon as a connection is detected. All the info, including username, time-stamp, IP address and so forth will have to be in a MySQL Database. At the end of every completed form a PASS/FAIL PDF report will need to be generated. In case of FAIL report, the report will have to include all the relevant failed fields and pictures. This PDF report will need to be e-mailed to multiple individuals in which contact e-mails will need to be stored or pulled from contacts stored on device. This report will also need to be stored and possibly modified at later dates. Let me know what info you need and how long it'll take to complete.
