Database with online form needs to be created

We currently create paperwork to document the client's requirements. The requirements vary greatly depending on the client's needs. Currently the paperwork is created in an excel spread sheet. This presents a number of difficulties, including the need to copy the same information multiple times. Also a lot of the form may not need to be completed if a particular service is not required. Once the form is completed it is printed for distribution to production.
