Business Directory - PHP MySQL

Please use [login to view URL] as an example I need a directory for businesses/people I need a page so that businesses/people can put their info in to our database I need them to be displayed in a list with the option to click on their individual profile. (Very similar to [login to view URL] zip code lookup section.) Internally, when I'm logged in: I need to be able to look them up by zip, then check a checkbox and press 'send email' and send an email to all of them. I need this to be commented very thoroughly The fields I need for the business/company are: FirstName LastName Email PhoneNumber (10digits eg, 6465551212) PhoneExtension PhoneNumber2 PhoneExtension2 Company Zip1 Zip2 Zip3 Zip4 Zip5 Zip6 Zip7 Zip8 Variable1 Variable2 Variable3 Variable4 Variable5 Variable6 Variable7 Variable8 Variable9 Variable10 (Zip is the zip codes they cover) ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
