Content delivery and SQL database quick job

On [login to view URL] (a Wordpress blog) there is a module on the right that asks people to fill in some fields and then download a guide. I need this done ASAP, ideally within 48 hours. Please respond to this bid with how quickly you can get the job done. I need someone to program this module so that when someone fills in the info and then hits "download now" the following happens: 1) They are forwarded to a URL where they can download the guide. 2) A SQL database entry is created with the information they filled in. That includes these fields: first name, last name, primary specialty, email. I also want to create a back end for the database where I can login and download a CSV of all the SQL entries. This needs to be password protected, and then there's just a simple "download" button. When I hit "download" I download a CSV of every piece of information ever recorded into the database. Again, I need this done ASAP, ideally within 48 hours. Please respond to this bid with how quickly you can get the job done.
