Centralisation of information for patient medication

I am looking for a program to be written that will centralise information inputted into 2 independent already existing software programs so that it can be seen remotely. The software will also automatically sync with the programs every 6-12 hours to ensure data is automatically kept current. The purpose of the software is to allow the patient's Doctor to view their current medication file and then using the template make changes that are then instantly transmitted to the pharmacy, in the form of a notification. The form should utilise pop ups to prompt the Doctor to input the required information and be able to sign the form digitally. The profiles should also be able to be exported as a .doc or PDF file as well as emailed directly from the form. All information needs to be private and hence some sort of login, or registry credentials must act as security. The database should be able to only display the patients that the particular prescriber cares for, as well as showing all patients of that pharmacy to the pharmacy. The end goal is to have this software be distributed with the existing 2 programs to multiple pharmacies.
