Accident Report Form

Looking to create a incident report form using PHP scripting. ? Must dump data to a 2005 Microsoft Sql server.? The database is configured and has two tables one table has 61 columns and the other is 8 colums.? The form will ask the user for Name, address, etc.? Would like the form to hide and show data when an item is selected from an drop down menu.? ? No checkboxes or radio buttons are needed. ? For example if the person injured is a employee it will show a set of certain fields and if they are a visitor or patient it will show a defined set of form fields.? Also will need the capability to using multiple fields.? For example if they need to add multiple witnesses or individuals.? Form validation is required, like date & time and phone number validaton.?
