ACI - Contact Us

The design of my website was recently completed. I am happy with the design but there is an unresolved problem with the "Contact Us" page. The Contact Us page design is also complete but requests for information are not being received at my email account. I have little to no knowledge of what may be causing the problem but, from my observations, it appears to be a PHP issue. My hosting service provider is eNom.com. I have confirmed with the host and by running tests that my email is functioning properly so I know the problem must be something else such as PHP configuration. I am looking for someone who has experience resolving this specific issue or someone with verifiable success with setting up web pages where users can request information by completing required fields and selecting among various types of request for information and/or typing a specific question. The user should then be able to "submit" that request for information and the request should then be delivered to my email account. I need the project completed within three days of commencement.
