Create MySQL DB with 100 fields and online secure data input page

Need a MySQL DB created on internet host server with around 100 fields initially. A secure web page template must also be created to allow a data entry worker to update the DB without having access to other parts of the website or to the DB itself. This is a prototype project which if successful may well require more work.
