Contact form - get it to work - 2nd attempt

$10 on offer to anyone who can quickly get the attached contact form to work - must be able to send to multiple addresses (to, cc and bcc) - trying to avoid having to recode the form. Has to be $10 as thats all I have in my account at the moment - but work with me on this and more work will follow. Obviously you will have to bid $30 but I'll send the $10 and then the remainder in a couple of days. If you need any further files associated to this file please ask.
