Based on your description and form image. I can turnaround the form pretty quickly. I also noticed that you will need the form sent to numerous people once submitted. I have experience with automating processes using workflows. I hope to hear from you in the near future.

I have a few questions for you. 1) Which SharePoint version are you using? SP2007 or SP2010 or SP2013? 2) Are you using the form that triggers a workflow? If yes, do you need a column that triggers the workflow condition? 3) After the form has been filled up and submitted by the reporting person, does it need to be "Read Only" columns when the form is sent to the supervisor?
