Contact Form For Wordpress Website

I have this website I'm working on [login to view URL] See that "Quick quote form" right by the slider in the home page?? I paid 20 dollars for that contact form. I need to add the same "New Client Quick Quote form" to the following pages but below the main body text. when Get Quote button is submitted from this specific pages, information is sent to 1 particular email.. - Colorado Auto Insurance - Colorado Home Insurance - Colorado Umbrella Insurance - Colorado Toy Insurance And I need to have a similar contact form for the pages below. However change the button from Get Quote to Get Info, and the information will go be sent to a different email... - Account Review - Billing Questions Thanks
