Custom Data Capture Websites

This is an ongoing project where one is required to be prompt, professional and provide excellent quality. What is required of you. I am looking for someone with examples of their work and are able to complete a "test" website upon request. All I'm looking for is the layout, which consists of the header, bg image and footer, no copyright services or anything else is needed. You must be able to provide me the website after 1 week. Send me your proposals...no emails...I will not respond to them, only through GetAFreelancer. In your response provide the price you are able to complete each website.
