Create a responsive, progressive web form -- 3

You are to create a responsive web form (using a pop up with jQuery-UI - compatible from mobile to 5k retina displays) that will validate before you continue to the next portion of the screen using jQuery Validate. Video of the web form this is to replicate has been included. Web form is to be an exact replica of the experience you see here. There will be one additional page after the 'get pricing' that will be a thank you screen, once the form has been successfully posted. You must also create a submitter (either through ajax or php) that will submit the form through a post submit, without navigating away from the page and display a Thank You once the job is done. A sample working form that is not progressive has also been attached, with the post url being scrubbed for privacy reasons. You are to use [login to view URL] as your template (including hidden fields, validation and scripts) and stylize it to create a like for like experience to the video. The submitted files are going to be 1 HTML file, 1 JavaScript file and 1 css file.
