Please be advised in advance. We reject bids generated automatically. We need to launch a website with simple forms initially. An index page with simple graphics and two pages with forms with captcha. The forms should write to a txt file in the server directory and email the input of the form to static email address. The initial website is only to meet immediate demand. As soon as the website is launched, we will begin the next phase with additional features and PHP/java scripting and database. Will provide cPanel access to webserver, database and PHP. The purpose of the site. We have been providing individual doctors, nurses and hospital staff with masks and gowns for 1 week at a time, when the hospital doesn't have supply. Doctors ask us for help because they only have 1 mask a day, and we find someone that wants to send them 10 or 20 masks for the week. We do not sell supplies and we do not charge anyone for anything. No one pays and no one collects money. Some people want to help but they don't have supplies, but they have small amounts of money. If they want to pay for the shipping and postage cost for someone that has supplies, we let people compensate givers for postage or freight. Attached is the basic three pages/forms we need to start.
