Custom HTML Templates

I am looking for someone to design 5-10 various HTML template files, along with a corresponding thumbnail image of what it would look like. These templates will match the header and footer sections of standard invoice/quote/sales order type of documents, but should not have that defined. I would like a variety of styles, both professional and basic, that will be presented for end users to be able to select and use. These should be stand alone plain HTML formatted text files that do not contain any reference to external CSS or any javascript.
