Develop a Mac Application

Dear Peter, Kindly consider this as a placeholder bid as we need to confirm our understanding and also have some questions : 1) We are assuming that the question set is limited (as provided with the attached PDF) and there is no need to change the questionnaire on the fly. Is this correct? 2) We have an understanding that the data will be saved locally on the Mac OSX computer and each different installation of this app will have its own data, such that there is no need of any kind of interconnection / data access between two or more separate installation of the app. Kindly confirm. 3) Once the form is filled and answers are captured, do we need to generate some kind of report (a document) for review and sending over the email ? 4) The attached PDF also talks about ongoing review mechanism after the initial risk assessment. Do we need to cater to this at this point in time? 5) You also talked about a website, is this presently in scope or if targeted to be developed separately?
