728 Photoshop form sliced and coded, PayPal integration

Existing JOOMLA site that was developed from a custom photoshop file now needs a one page form coded and integrated into the site. The form is also completely ready in photoshop (NO design needed just slice and code) and the form needs to be integrated with PayPal PRO. It is a donation form where a user can select categories to donate to and enter an amount. the amount needs to be totaled and sent to PayPal pro for processing. Complete photoshop file will be provided - the rest of the template and site is complete so really it is only slicing and coding the form itself and then integrating with PayPal PRO.
