Wirte a MongoDB Aggregation Function

I need help writing a MongoDB aggregation function. My app is used node, express, and mongoose. The output of this aggregation will display in a table. Below is the criteria for the aggregation, an example of the how the data will display in the table. Attached is a sample document that the aggregation will utilize. ******ACCESS TO OUR DB AND CODE BASE WILL NOT BE GRANTED.
