Microsoft Word document automation

I am looking for someone to help me automate a Word document/template which I use to generate proposals. The template document always stays the same except for 18 variable elements in the document. What I am envisioning is a form with - * 12 single line text fields * 2 paragraph text field * 4 multiple checkbox selections When the form is populated, the inputs/selections are automatically populated in template in Word and the document can be then saved as a complete file. It would be necessary also to edit the text that is generated by the selection of checkboxes. I am doing some of this using 'Find and Replace" but need something more effective.
