I have a new Adobe Illustrator graphic I need sliced and put up as a template, and new pages created from it. It's light on coding, just php forms really, and some CSS.

4. Create HTML content template
a. The content pages template should feature the same header (menu/phone number), have a different image on the header, and have the right hand contact us form on all pages.
b. The left hand box, with the services should be removed, so content begins on top on the left and continues on the bottom

b. On all forms make the die page be “[login to view URL]” – it should look the same as template, and in the text write “thank you for your submission”
c. On email make sure subject of email with submission is very accurate “ONLINE LEAD” for main form, “ONLINE TESTIMONIAL” for testimonial form, and “ONLINE CONTACT” for contact us form.
