Joomla - PHP - Component

I need a simple PHP application that inserts information into a MySQL database.? You do not need to create the database.? I will do that. This would be a phonebook type application.? Usual fields plus a couple more.? ID (key), First name, Last name, second last name (for latin america), age, gender, phone1, phone2, cel1, cel2, fax, province, county, district, neighbourhood, other1, other2, other3, other4. I need five people to be able to input data.? Only 2 people can delete data.? -If, and only IF this makes it easier, you may program it like a component for Joomla 1.5 or 1.0. ID is numeric, must be unique, and MUST BE 9 digits. I would like to be able to pull reports out of this, by any of the fields described, and if possible, export to CSV. Worry not at all about design.? Simple form will do.
