Fix CSS and Change an HTML page into PHP with include or require functions for header and footer

On my site, the top blue graphic is not appearing in the heading. It only appears in the footer. It works locally on my computer, but it's not working when all of the files are uploaded. I believe it is a minor CSS issue, but I don't have the time to find the issue and fix it. I need someone to tell me how to fix it. The site is: unik test .com, (but with no spaces.) Additionally, I want someone to transform the homepage from HTML to PHP. I want the header and the footers to be in separate include or require function files so I only have to edit it in one place instead of on each page. The page is already designed responsively, so I don't need help with that. I will use the php layout you've done for the header and footer of the homepage to apply to the other pages of the site. If you can quickly fix this, there is potential for me to hire you for other small CSS microprojects to help me complete this website within the next few days. I need this done very quickly - within the next few hours. This should be very simple and easy for someone who knows PHP and CSS. Also, I won't prepay.
