Set up ClickBank Advanced Custom CSS Order Form - open to bidding

Hi Ming, I have the website [login to view URL] which helps people stop drinking or moderate their drinking. It works with ClickBank and at the moment if you go to the order form (please click on the Add to Cart button) and you will see I use the basic ClickBank Order Form customization which is basically just a header there. I would like to trial one or two different variations of custom order forms using testimonials, the logo, copy to encourage people to buy, maybe a short video (I am working on a video sales letter pitch page so maybe a shorter version of this). The same coloring and design, 365 day logo guarantee, Doctor Trusted Certificate etc. Basically things which are already on the sales letter. At the moment, I only have around a 5% conversion rate of those people who go to the order form and then buy. Please let me know your thoughts on this and how to best provide you with the content. And any suggestions or templates you can recommend looking at for good custom order forms. Thanks, Rahul
