Dynamic HTML sales page required (Example in description)

I want something that can do this: [login to view URL] Ie... you put in some details like first name and last name on the landing page. Then when you click the submit button, it takes you to a sales page that uses those details to make it a more personal experience. Note how images are also personalized, I would want this too. I have experience with designing web pages with WordPress and basic HTML. On completion, I basically want things in place so that I could create the same set up as the example 2 pages in the example but with my own text and images (Landing page and sales page). I will obviously write the sales copy. I just need you to send me the 2 pages in HTML with instructions on how I customize them. Also - I need to be able to recreate the look and feel of both the landing page and sales pages. Landing page: So I can use my own images and styling. Sales page: Include example with enough dummy text, to allow me to copy and paste it multiple times and reproduce a long sales page, with the same layout as theirs. Including an example personalised image (Ie. containing first and last name) and 2 containers that have a white and grey background (Also, I need to be able to change the 2 containers colors if I want to). Please ask anything that you think I've missed. Should be a quick project for someone with these basic programming skills.
