Square Payment Gateway/Membership Digital Form Improvement

We need assistance integration square payment button at the bottom of the "become a member" page. We want website visitors to be able to fill out the registration form, pay $50 year membership fee, then email is sent to them and admin also if possible, we would like for the user name, email address, be taken from the form and placed into a database of some sort. Goal is to be able to see and track and store how many members sign up and have the ability to export membership data for committee/administration use. Also for email marketing purposes. I have attached screenshot of how the form is currently setup. ideal candidates has experience creating, designing and implementing user friendly forms. Its important the form is easy to use as the membership base that will be using is older in age.
