Simple website with graphics, forms, php, comments

You will be required to upload and install the full running website. The site is quite simple. Visitors will enter their name and a date and hit submit. The Name and Date will then be displayed to the user on a results page within a URL. IE: domain.com/n=Sandra&month=Jan&day=31&year=2008 Then they will be able to enter 6 email addresses to invite friends to view their page at their new URL. The page at their new URL will use the name and date fields to auto-fill a short joke document with the name and dates in relevant places. You will need to design the website to include tasteful graphics. You will need to incorporate Adsense ads into the website. Visitors to the website should be able to leave comments using a gif-protected comment form. See the attached Word doc for details. Any bidders who have not read the attached Word doc will not be considered.
