Wordpress Conditional RSVP Form Driven by an Excel File

I am looking for someone to develop a custom conditional form for my wedding website. I need to build a custom RSVP tool for my Wordpress wedding website that is driven from an excel file (sample attached; happy to port the excel file to another format as necessary). The form will initially ask the visitor to enter their unique identifier, and based on that, query the excel file and present the appropriate set of drop down options for the just the events that guest is invited to.
