we are looking for somebody to create one full page on our website which has the same form as the one in the uploaded files. we know that the formidableforms has been used to create the required form, because we have no backend access to that website we need somebody to create the form using the formidableforms plugin. the form should be fully functional and send answers via smtp to our email. the form has to use conditional logic
