Basic Online Quiz Engine

I need a simple quiz program written in PHP. All details are as below, subject to minor changes along the way. The main features are: ---------------------- 1. Portable and self-contained in one folder/directory 2. Does not require DB or user registration 3. desktop and mobile friendly Quiz functions: --------------- 1. User can fill in personal info & captcha before the quiz begins 2. Quiz type: Multiple choice (single answer), Multiple choice (multiple answer), text-based answer or a combination of these 3. User can continue unfinished quiz at a later time as long as he didn't clear browser cookies 4. Simple reporting 5. Prevent spam with captcha before the quiz begins Quiz designer functions ----------------------- 1. Simple admin login 2. add new, edit and delete questions, and re-order sequence (or set to random). Each question has a check box to publish or not publish. 3. each question has option to upload/insert pictures or URL (e.g. for youtube video), if URL is youtube video, the video will be embedded into the question. [login to view URL] acceptable answer or wildcard 5. option to allow user to view answer/result after each question, or at the end of quiz, or not at all. Result will be saved as text file, and also emailed to user/admin after every quiz is completed. Example Quiz Flow: ------------------ page 1. User see a static page of the quiz background information page 2. User optionally fill in personal info, then enter captcha to enter quiz page 3 - page n. User works on the quiz, one question per page. can go back/forward if needed. Incompleted quiz will still be accepted. After each question, user may see his score, his answer and the correct answer (if it's a multiple choice question) Page n+1. HTML thank you page and final score (for multiple choice) and a simple report. User can request report to be sent to him. A copy of the results will be sent to admin. 5. the quiz uses a simple layout/theme which can be manually edited to a certain degree.
