Creation of Form user interface , and output of charts and l

Excel 2003 VBA task to create a form entry front page which provides certain criteria for which to search another spreadhseet for certain data (spreadsheet is dynamic in that all columns stay the same but rows will be frequently appended). From the search criteria a Pareto chart will be produced for given criteria over a form entered period of time. The tool will also provide another chart showing the frequency of given criteria over a form entered period of time. A list also to be produced (can be a newly created worksheet)listing certain data entry rows that meet form enetered criteria.
