Login-Page for Hotspot

For a hotspot I have two pages actually, one bare-bone for the login procedure (it works already), written in HTML+Javascript, which is simple text plus two images (logos), AND a second one, beautifully designed, which actually only contains quite some beautiful graphics and some fixed text. These two ones have to be merged, to have a beautiful login-page. Actually, on the first simple page, user has to type in his name only, which in fact is either "EN" to signal English language, or "RU" for Russian, to signal Russian language. On the "Merged" login-page, user instead should only check 2 out of three check-boxes: - either check-box for "English Language" or "Russian Language" - Acceptance of "Terms of Use", so there must be a link to another page, stating "Terms of Use" in two languages, simultaneously. And press the OK-button, the "OK" has to switch to Russian, if necessary. Because of the special hardware used, remote testing by the supplier will not be possible. I have to do it, and supplier should be available online via ICQ, for example. Supplier might think about some log-files or similar, to make testing for me easier, because I do not know Javascript, and only basics of HTML.
