Form Page Frontend Restyling (Web App)

# Starting point: A simple web app to submit forms. - LAMP stack. - Has a page for login, and a page for the form. - Form can be accessed by logging in as a registered user. - Form has fields that a user needs to fill in by clicking on the answers. - Form has no text. - Form uses images next to the radio buttons to indicate answers. - Repository on Bitbucket. # Deliverables: A bug-free working code that must be able to: - Change the radio buttons completely into the images. - Check/select each radio button by clicking at the image, changing the newly-selected one from grayscale to colored version. - Uncheck/unselect that radio button by clicking at that image, changing the newly-unselected one from colored back to grayscale version. - Check/select another button by clicking at another image, apart from the previously-selected one, changing the newly-selected one from grayscale to colored version, while also therefore uncheck/unselect previously-selected button and changing its image back to the grayscale version. - Have the clean layout that looks like the screenshots. - Have the responsive look that doesn't stretches or breaks on mobile browsers. - Have the images able to clicked easily. # Requirements: - Must be willing to have meetings to discuss the UI/UX. - Must be proactive in asking questions of the boundaries of the deliverables. - Development must be done on a Bitbucket repository that will be given. - Branches of code must indicate each feature. - Commits of code must indicate small change of code. - Code will be reviewed daily to see the progress, direction, and code style. - Revisions of the deliverables will be done to avoid confusions and misdirection.
